Based on visual observation the facility failed to maintain the rated doors in barrier. The deficient practice would allow for smoke and fire to travel from one barrier to another and affect the exiting discharge in the event of a fire or smoke emergency affect 10 of the 10 residents of the facility.

Finding:
1) - During the facility tour on 07/13/2011, between the hours of 8:30am and 12:00pm, the rated corridor doors in the smoke barriers through-out the facility were being propped open with wood material causing the doors to be allowed to stay in the open position.

Based on visual observation the facility failed to maintain the separation of hazardous areas from other parts of the building, including the egress corridor. Hazardous areas are required to be constructed to resist the passage of smoke. The deficient practice had the potential to affect 10 of 10 residents. 1 of 3 smoke compartments have hazardous areas that are not separated.

Findings:

1) - During the facility tour, between the hours of 8:30am and 12:00pm, it was observed that the door leading into the medical records storage room was not equipped with a self-closing device.
2) - During the facility tour, between the hours of 8:30am and 12:00pm, it was observed that the walls in the medical records do not extend to the roof deck.
3) - During the facility tour, between the hours of 8:30am and 12:00pm, it was observed there are several 3/4" inch to 1" inch holes penetrating the three (3) interior walls and several 1/4" inch holes and a large 2' X 4' foot opening in the ceiling that are not sealed to maintain the rating of the room.

Based on visual observation the facility failed to provide 1 of 4 exits that are accessible. This would allow for the exits to not be used in the case of an emergency and potentially affecting 10 of the 10 residents of the facility.

Findings:

1) - During facility tour on 07/13/2011 between the hours of 8:30am and 12:00pm, the double exit doors by the service entrance, located on the southside of the facility would not opening on the left side because of the hinges on the door are broken and causing the door to not open.

Based on visual observation the facility failed to assure that the fire alarm system was installed, inspected and tested in accordance with the Life Safety Code and NFPA 72. The fire alarm system gives a sense of security to offer an advance warning in fire and/or smoke emergency. This deficient practice could potentially affect 10 of 10 residents of the facility.

Findings:
During the facility tour on 07/13/2011, between the hours of 8:30am and 12:00pm, the strobes through-out the facility for the fire alarm system are not in synchronization in accordance with NFPA-72

Based on visual observation, the facility failed to assure that the means of egress was free of obstructions or impediments to full instant use of the exit passage way. Obstructions, in the egress corridor, hinder occupant egress in emergency situations. This deficient practice could potentially affect 10 of the 10 residents in the facility. Facility had 3 of the 3 corridors that were deficient.

Findings:
During the facility tour on 07/13/2011, between the hours of 8:30am and 12:00pm, corridors have medical equipment and storage such as patient lifts, linen carts, severing charts, chairs and wheelchairs . That are being stored in the corridor for more than 30 minutes.

Based on visual observation the facility failed to maintain the electrical wiring in accordance with NFPA-70. The deficient practice would delay the response to shut off power or work on in the event of an emergency and affect 10 of the 10 residents of the facility.

Finding:
1) - During the facility tour on 07/13/2011, between the hours of 8:30am and 12:00pm, the four (4) electrical panels in the boiler room have storage placed in front of them.
2) - During the facility tour on 07/13/2011, between the hours of 8:30am and 12:00, the electrical transformer in the boiler room had storage such as gas lawn equipment and milk crates that were stored with-in the allowed requirements.
