Based on review of facility documents and staff interview, (EMP), it was determined that the medical staff failed to complete patient medical records within 30 days of discharge for two of fourteen discharged medical records reviewed. (MR1 and MR13).
Findings include:
Review on July 22, 2020 at approximately 2:00 PM of the facility policy, "Authentication of Medical Records Policy # 700.8 " dated January 2, 2020, revealed, "... Records of patients discharged shall be completed within 30 days following the date of discharge."
1. Review of MR1 on July 22, 2020, at approximately 1:45 PM revealed that the patient was discharged on December 30, 2019. Further review of MR1 revealed that the discharge summary was not completed by the nurse practitioner until February 11, 2020. The document was not cosigned by the physician until June 13, 2020.
2. Review of MR13 on July 22, 2020, at approximately 2:30 PM revealed that the patient was discharged on May 13, 2020. Further review of MR13 revealed that the discharge summary was not completed by the nurse practitioner until June 25, 2020. The document was not cosigned by the physician until July 13, 2020.
During an interview on July 22, 2020, at approximately 2:30 PM EMP8 confirmed that the discharge summaries for MR1 and MR13 were not completed withing 30 days.

REGULAR FIRE AND SAFETY INSPECTIONS

Based on review of facility policy and staff interviews (EMP), it was determined that the facility failed to request an annual inspection by its local fire department.

Findings include:

Review on July 17, 2020, at approximatley 11:00 AM of the Facility policy "Condition F ... FIRE ... General Fire Safety ... Last reviewed and updated 1/2018" revealed, "... The Children's Home of Pittsburgh has established the General Fire Safety policy to help reduce or eliminate the potential for fires to occur as well as to protect the lives and well-being of all staff and visitors in the event of a fire. ... "

Review of facility documentation revealed no evidence of a request to the local fire department to perform an annual inspection in calendar years 2018 and 2019. Further review revealed no fire inspections by the local fire department during the years of 2018, 2019, and 2020.

During an interview on July 17, 2020, at approximately 11:00 AM, EMP6 confirmed the facility did not request the local fire department perform an annual inspection in calendar years 2018 and 2019.
