1) Records documented repairs were made to the system on 4/20/10. The was no documentation that the entire system was re-certified and did not list the technician performing the work.

Based on review of the records (weekly generator inspection form) of tests; including, the weeks of 8-2, 8-9 , 8-16, 8-23, 8-30-10, and previous weeks, the facility failed to exercise the generator under load for 30 minutes monthly in accordance with NFPA 99 Section 3.4.4.1.1.
