Based on review of policies and procedures and interview, it was determined the facility failed to assure patients, staff and visitors were free from abuse while on the premises. The failed practice did not assure corrective actions would be taken if anyone witnessed any form of abuse to patients, staff or visitors in the facility. This failed practice had the potential to affect all persons in the facility. Findings follow:

A. Review of policy, "Patient Rights and Responsibilities" revealed no evidence of actions
(i.e. reporting the event, removing the abusive person from the suspected abused person's environment, etc.) to be taken when witnessing an abusive event or situation on the premises.

B. During interview with the Director of Accreditation/Regulatory Compliance on 08/23/13 at 1400, she confirmed the findings.
