Based on document review, observation and interview, the facility failed to inform each patient of their patient rights.
Findings Include:
1. Review of policy titled: Patient Rights and Responsibilities last approved 09/2018 indicated that all patients are informed in writing of their rights and responsibilities.
2. This surveyor asked the Registration Clerk for the written Patient Rights given to the patients. The surveyor was handed a pamphlet titled Joint Notice of Privacy Practices. Upon receiving, this surveyor read the contents which did not include patient rights.
3 Tour of the Registration area on 04/07/21 at 9:45 am, surveyor noted when at the Registration area that the facility had posted Patient Right's but it was not in an area that could be visualized by all patients.
4. Interview on 04/07/21 with P50 (Director of Quality) and P51 (Clinical Nurse Specialist) at 10:45 am, each confirmed that the pamphlet titled Joint Notice of Privacy Practices did not contain a list of patient rights.
