Based upon record review and interview, the hospital failed to maintain the list of contract services. This was evidenced by the failure to identify on the list 6 out of 11 contracts that were furnished under agreement. Findings:
Review of the list of contracts revealed 5 agreements were identified. During the tour of the Radiology Department on 02/27/24 at 10:10 a.m. interview with Staff D revealed Contract K was utilized for radiological equipment repair and Contract N for Dosimetry.
On 02/27/24 at 1:40 p.m. interview with Staff A revealed when asked if there were contracts for K and N which were identified by Staff D these contracts were submitted for review along with Contracts L, M, O, and P.
Contracts K, L, M, N, O, and P were not identified on the list of contract service agreements.
