Based on observation and staff interview, the facility failed to meet the provisions of the applicable 2012 edition of the, " Life Safety Code of the NFPA (National Fire Protection Association) 101." This deficient practice could affect all patients, staff and visitors in the areas referenced. Facility Census: Nine (9).
Findings include:
1. In reference to the Federal Life Safety Code, the facility failed to maintain the building in a manner to ensure the safety and well being of patients, staff and visitors. Stained ceiling tile were observed in:
A. Stained ceiling tile in the kitchen above the coffee maker.
B. Stained ceiling tile in the kitchen next to the director office.
C. Stained ceiling tile in the cafeteria behind the cash register.
D. Stained ceiling tile on the first floor in the laboratory.
E. Stained ceiling tile on the 3rd floor next to room 303.
F. Stained ceiling tile on the 3rd floor next to room 304.
G. Stained ceiling tile on the 4th floor next to the orthopedic room above the work table.
2. The aforementioned deficiency was discussed with the Administrator and the Maintenance Supervisor on 09/13/17 at approximately 4:00 p.m. and agreed this deficiency needed corrected.
