Based on a tour of the facility and interview with staff the facility failed to clean point of care equipment (glucometer) according to manufacturer's guidelines. The findings include:
During a tour of the adult unit with the Assistant Nurse Manager on 5/11/15, glucometer disinfection was reviewed with Licensed Practical Nurse (LPN) #1. Upon surveyor inquiry LPN#1 indicated after each patient use the glucometer is cleaned with isopropyl alcohol. Review of the glucometer manufacturer's guidelines for cleaning and disinfecting indicated an Environmental Protection Agency (EPA) approved cleaning (Super Sani-wipes) solution was to be used to cleanse and disinfect the glucometer. In review of the manufacturer's guidelines with the Assistant Nurse Manager he/she was not aware of the manufacturer's guidelines for cleaning and disinfecting the glucometer.
Facility policy for Cleaning and Disinfection of the Accu-chek Aviva Glucometer identified the glucometer was to be cleansed with Super Sani-cloths however the policy further indicated 70% rubbing alcohol, mild dishwashing liquid mixed with water and/or 10% household bleach (1 part bleach plus 9 parts water) made the same day could also be used to cleanse and disinfect the glucometer.
