Based on observation and interview the facility failed to ensure that all patient room doors had positive latching devices and that roller latches were not used on corridor doors. This affected all 20 patients residing in the behavioral health unit. The facility census was 29.
Findings included:
1. Observation on 08/29/11 of the behavioral health unit showed rooms 302, 303, 322, 323, 325, 326, 327, 328, 329, 330, and 331 to have only roller latches, which do not latch securely, on the corridor door to the rooms.
2. During an interview on 08/29/11 at 3:10 PM Staff N, Director of Plant Operations, stated that all 11 rooms in this portion of the unit have roller latches in place on the corridor doors.

Based on observation the facility failed to ensure that all cylinders of compressed medical gasses were individually secured. The facility census was 29.
Findings included:
1. Observation on 08/30/11 at 11:10 AM showed the outside storage area for compressed medical gasses had three metal cages which contained cylinders of compressed medical gasses.
-The first cage contained 39 "E" size (approximately 25 inches in height) secured by one chain across the front of the group of tanks.
-The second cage contained 33 "E" size tanks secured by one chain across the front of the group of tanks.
-The third cage contained 25 tanks of various sizes with no chain or other method of securing the tanks individually.

2. Review of Chapter 5.1.3.3.2 of the 2002 edition of the National Fire Protection Association Code 99, Health Care Facilities, states that locations for central supply systems and the storage of medical gases shall meet the following requirements:
(7) be provided with racks, chains, or other fastenings to individually secure all cylinders, whether connected, unconnected, full, or empty, from falling.
