Based on interview and record review the facility failed to ensure that an annual inspection of fire doors was done at the facility. The facility census was zero Acute Care patients, five Swing Bed patients, and three Observation patients for a total census of eight.

Findings included:

1. During an interview on 08/16/23 at 9:30 AM, Staff I, Director of Quality, stated that an inspection of fire doors had not been done at the facility.

2. Review of the facility inspection records did not show an inspection of fire doors other than on the fire alarm test document which showed they closed but did not include any other criteria for a fire door inspection.

Findings included:

1. During an interview on 08/15/23 at 4:30 PM, Staff I, Director of Quality, stated neither maintenance nor bio-medical was doing an inspection of electrical outlets. Staff I was unsure if a policy existed for this inspection and testing.
