Based on record review and interview, the provider failed to ensure the patient has the right to be free from all forms of abuse by failing to provide information regarding abuse and neglect (to include reporting requirements, prevention, intervention and detection) during the orientation training for 2 of 6 employees whose personnel files were reviewed (S2MHT, S3MHT). Findings:
Review of the personnel files revealed S2MHT was hired 08/17/2024 and S3MHT was hired 12/17/2024. Review of the training logs for S2MHT and S3MHT revealed no documented evidence of training related to abuse and neglect.
On 01/08/2025 at 12:50PM, an interview with S1ADM confirmed that the above employees had not received training regarding abuse and neglect during their orientation and had not received training as of this date.
