Based on document review and staff interview, the facility failed to provide documented evidence of a risk assessment in accordance with NFPA 99.
Findings include:
During document review it was found that there was no risk assessment performed pertaining to the building system risk categories, as described in Chapter 4 of NFPA 99. On 5/30/18 at 11:30 AM the Director for Support Services was asked for this assessment, and indicated such a written assessment had not been completed.

Based on observation, document review, and staff interview the operating rooms were not served by isolated power or ground-fault circuit interrupters.
Findings include:
On 5/29/18 at 10:30 AM while touring the operating rooms, it was observed that the operating rooms did not appear to have isolated power devices or ground-fault circuit interrupters. On 5/30/18 at 11:30 AM the Director for Support Services was interviewed and indicated the operating rooms are not considered wet locations due to the types of procedures performed, but a written record of this assessment in accordance with NFPA 99 was not completed.
