Based on clinical record review and interview, it was determined the Facility failed to disclose the risks and benefits of transfer specific to the condition for 15 (#3, #5-#9, #11-#13, #16-#20 and #23) of 15 transferred patients. Failure to disclose the risks and benefits specific to the condition did not allow Patient #3, #5-#9, #11-#13, #16-#20 and #23 to make an informed decision regarding the need for the transfer. The failed practice affected Patients #3, #5-#9, #11-#13, #16-#20 and #23. A. Review of the clinical records of Patients #3, #5-#9, #11-#13, #16-#20 and #23 revealed no documentation of the risk and benefits specific to the condition for transfer. B. Registered Nurse #2, in the company of the Vice President of Quality, reviewed the clinical records for Patient #3, #5-#9, #11-#13, #16-#20 and #23 on 08/06/15 from 1545 through 1630 and verified that each clinical record lacked documentation of the risk and benefits specific to the condition for each transferred patient. |
