|Based on interview and record review, the facility failed to ensure that staff documented in the patient's medical record that the list of available home health agencies was presented to the patient or to the individual acting on the patient's behalf in 4 of 28 sampled patients (#16, 17, 18 & 19).
A review of the medical records of patients #16, 17, 18, 19 was performed. All had orders for home health at discharge. In none of the records was there any evidence that the patients or caregivers had been provided with a list of available home health agencies.
During an interview of the Risk Manager and the Administrative Director - Case Management on 8/20/12 at 5 p.m., they confirmed the preceding.
