|Based on observation, document review, and Staff interview, it was determined that the facility failed to ensure compliance with the National Fire Protection Association's Life Safety Code, 2012 edition.

1. On 12/21/16 at 2:30 PM, the pedestrian stairs, in the parking deck, were painted a flat gray which obscured any indication of step location.
