Based on record review the hospital failed to provide an accurate prescription to the patient at discharge.

Findings:

On 12/05/2014 the patient was dischrged from the hospital to a skilled nursing facility. During the patient's admission, the patient had been followed by physical therapy who assessed that it was necessary that the patient have a wheeled walker. The patient would need to be discharged with a prescription for a wheeled walker, because prior to admission the patient was independent with ambulation.

On 12/03/2014 a prescription was written for a wheeled walker. The prescription was written in the name of another patient.
