Additional Survey Findings: Based on personnel record review, facility policy and procedures, and interview revealed 5 out of 8 employees did not have a record of their annual health evaluation in 2014. Additionally, the same employees did not have a current 2014 Mantoux test for tuberculosis. Findings include: Review of staff members ' health records on 3/10/15 revealed 5 out of 8 employees did not have a record of an annual health evaluation in 2014. Additionally, 5 out of 8 employees did not have a current Mantoux Test for 2014. (Staff #3, 12, 13, 14, 15). Interview with Staff #7 System Director Health Information on 3/10/15 revealed documentation of an annual health assessment and Mantoux Test was not present for Staff # 3, 12, 13, 14 and 15. Review of Policy Title Employee Health Services: Policy #HR.600, Date of revision 3/13. B. Employee Annual Physical Reassessment. Each employee ' s health status will be reassessed yearly on the anniversary of their employment and will include the following. 1. Health questionnaire to update the employee ' s established medical history. 2. Mantoux test for tuberculosis If the annual reassessment form is not returned within the allotted time, a notice will be sent to the employee supervisor to follow the disciplinary process and suspend the employee until they complete the annual reassessment process according to policy #HR250 |
