Based on staff interview and record review, the facility failed to obtain informed consent for 1 of 10 patients in the sample (Pt # 6). Findings include: Per record review on 10/12/11 at 9:25 A.M., there was no informed consent obtained for an invasive surgical procedure for patient # 6. Per review of a physician's procedure note dated 8/21/11, patient #6 had a chest tube placed on 8/21/11 at 11:43 A.M.. Patient # 6 was alert and oriented per review of the discharge summary. On 10/12/11 at 9:53 A.M. the Director of Regulatory compliance confirmed that there was no evidence of an informed consent for placement of the chest tube per facility policy. |
