Based on record review, and interview the hospital failed to ensure that a registered nursed supervised and evaluated the nursing care for 3 of 4 patients, (Patient #3, Patient#11, Patient#13) and reassessed the patients for injuries after being involved in an altercation.


Findings including

During the record review Patient #3, Patient #11, and Patient #13, records failed to ensure that a medical assessment was completed after the patients were involved in an altercation.

During an interview with Personnel#3, and Personnel #4, they were able to verify the patient's records did not reflect that a medical assessment or reassessment was completed.

The hospital Nursing Patient Assessment/Reassessment dated ,d+[DATE] reflected, "To provide the scope of patient assessments performed by nursing and other clinical disciplines ...Patients will be reassessed as needed or according to established protocol. Patients will be reassessed when ...identifies any change in patient symptoms."
