VIOLATION: PATIENT RIGHTS: FREE FROM ABUSE/HARASSMENT | Tag No: A0145 | |
Based on record reviews and interview, the hospital failed to ensure that patients were free from abuse for two (2) of ten (10) patients reviewed (Patient #1 and #2). Findings includes: 1. A "Reportable Incident Form for Certified, Licensed or Registered Providers", dated June 1, 2017, indicated that on May 30, 2017 at 7:05 AM Registered Nurse (RN) #1 grabbed Patient #1 by the arm and forcefully pulled the patient off the bed onto the floor. The patient was crying that he/she was hurt and scared. This incident was observed by Psychiatric Technician #1. A written statement by Psychiatric Technician #1, dated May 30, 2017, indicated RN #1 had grabbed Patient #1 by the arm and forcefully flung him/her off the bed. RN #1 stated "Get [him/her] out of here and lock me up." The patient was crying and stated he/she "hurt" when his/her arm was touched immediately after this incident. 2. An incident report, dated May 30, 2017, indicated that at approximately 6:45 AM RN #1 pushed Patient #2 off a counter with both hands. The patient fell to the floor and hit his/her left knee. This incident was observed by RN #2 and Psychiatric Technician #2. The facility's "Fact Finding Process" form, dated May 30, 2017, indicated RN #2 and Psychiatric Technician #2 both reported they witnessed Patient #2 on the counter. RN #1 pushed the patient off the counter, with both of his hands, and the patient fell on to the floor hitting his/her left knee. On June 5, 2017 at approximately 2:00 PM., the information above was confirmed by the Director Performance Improvement and Compliance and the Vice President and Chief Nursing Officer. |
