Based on review, observation, and interviews the Governing Body (GB) failed to retain a physician to direct Respiratory Care Services.

Review of credentialing files, policy, and procedures, and staff interviews the Director of Respiratory Services had resigned 7/1/13.

An interview was conducted with staff# 1 on 10/2/13. Staff #1 confirmed that the Respiratory Department did not have a director since 7/1/13.
