COLUMBIA MEMORIAL HOSPITAL 71 PROSPECT AVENUE HUDSON, NY 12534 March 12, 2018
VIOLATION: FACILITIES, SUPPLIES, EQUIPMENT MAINTENANCE Tag No: A0724
Based on observation and interview the facility failed to ensure that "crash carts" containing emergency equipment for patient emergencies were fully stocked on 1 of 3 inpatient units (Fifth Floor) inspected during the Department of Health survey. Findings:

-On 3/9/2018 at 2:45 PM, survey staff inspected the contents of the emergency cart on the Behavioral Health Unit. The emergency cart lacked an Ambu bag.

-Survey staff interviewed the Director of Psychiatry and the Facility Risk Manager at the time the observation was made; staff verified that the Ambu bag was missing from the cart and verified that the facility requires this item to be on all their emergency carts.
