VIOLATION: MAINTENANCE OF PHYSICAL PLANT Tag No: A0701
Based on observations and interview, the facility failed to ensure that dirt, grit (particles), and grime (dirt adhered to a surface) were not accumulated on floors and base coving (creates a watertight transition between the wall and the floor) on the ground and second floors used for patients care. This deficient practice had the potential for the spread of infection and to affect the residents' comfort.

Findings:

On 08/20/2019, between 12 p.m. and 1 p.m., during a tour of the facility, accumulation of dirt, grit, and grime on the floors, base coving, and walls were observed in the following locations:

a) on the ground floor emergency room area, Unit 17 area, and in room 1; and,
b) on the second floor, in the pre-operation's stalls area.

During concurrent interviews with the administrative staff, the administrative staff confirmed that the floors, base covings and walls had accumulation of dirt, grit and/or grime.

A review of the policy, scope of services, effective December 2017, indicated the Environmental Services Department (EVS) ensures a clean and safe working environment for patients, visitors and staff by meeting the needs and requirements of our customers and governing regulatory agencies in a conscientious and professional manner. The EVS Management TEAM is responsible for the aesthetic maintenance and cleaning of all campus buildings. Hours of operation are twenty-four hours a day, seven days a week.
