WEST OAKS HOSPITAL 6500 HORNWOOD HOUSTON, TX 77074 Nov. 4, 2019
VIOLATION: PATIENT RIGHTS: FREE FROM ABUSE/HARASSMENT Tag No: A0145
Based on interview and record review, nursing staff failed to ensure patient (ID#5) was free from all forms of abuse or harassment in one (1) patient out of 22 sampled patients within the facility. The X-ray request was not faxed timely and patient (ID#5) did not receive the X-ray prior to discharge. Subsequently, patient (ID#5) was found to have a concussion as a result of the altercation.

Findings Included:

Record review of the facility policy on "Nursing Documentation", dated 11/2018, stated:
In accordance with the Texas Nurse Practice Act, nursing documentation accurately and completely accounts for
nursing care rendered; any changes in the patient's status (to include behavioral, physical, emotional, special precaution, and/or variances in care or outcomes)

Record review of provider notes dated 07/30/2019 at 0915 documented by the Nurse Practitioner, (ID#72) revealed patient (ID#5) was in an altercation and was hit in the face and nose by another patient and a X-ray was ordered.

Record review of nurses noted dated 07/30/2019 at 1344 by staff nurse, RN (ID#70) who stated aggression, was verbally aggressive toward a peer, and got into a physical fight. She was hit in the face several times, by the peer.

Record review of the discharge safety plan was signed by the CPS staff at 07/20/2019 at 1330.

Interview with Director of Quality (ID#51) on 11/04/2019 at 1055 who was reviewing the medical record during this interview stated:
She might have not gotten an X-ray, it was not ordered stat.
The x-ray was ordered 07/30/2019 by (ID#72) Nurse Practitioner,
X-ray would not come before four (4) hours because it was not a stat, before discharge.
