VIOLATION: INFECTION CONTROL OFFICER RESPONSIBILITIES Tag No: A0749
Based on document review, observation and interview, the facility failed to provide consistent screening of all visitors/persons entering the hospital by hospital personnel Specifically, hospital personnel at the main entrance of the hospital did not screen all visitors/persons entering the hospital for COVID-19 symptoms. This inconsistent practice may result in the spread of COVID-19.

Findings include:

-- Review of the hospital's "Visitor Guidelines," dated 5/1/2020, indicated all visitors will be screened by "Visitor Management" or reception staff to include questioning regarding; presence of fever, sore throat, runny nose, cough, shortness of breath, muscle aches or diarrhea and close contact with anyone who has been laboratory-confirmed as positive for COVID-19.

-- Review of the hospital's "Outpatient/Visitor Screening," algorithm, revised 5/6/2020, instructed "Visitor Management" to ask a patient/visitor if they have experienced any cold or flu-like symptoms in the last 14 days such as: fever, sore throat, cough, shortness of breath.

-- Per observation at hospital's entrance on 6/17/2020 at 9:00 am, "Visitor Management" did not screen a visitor/person by asking if he had any of the above symptoms e.g., fever, sore throat ... . The visitor confirmed he was not asked any screening questions.

-- Also, per observation at the hospital's main entrance on 6/17/2020 at 9:45 am, 2 people presented to "Visitor Management" and only 1 person was asked if they had any of the above symptoms e.g., fever, sore throat ... .

-- Per interview of Staff A, Senior Director of Support Services, on 6/17/2020 at 9:50 am, he/she stated the expectation is for hospital personnel to screen all persons presenting to the hospital by asking the above screening questions. He/she acknowledged the above findings.
