Based on medical record review and interview, the hospital did not meet the requirement because the medical record for Patient #1 did not include a discharge summary.

Findings Include:

Review of the medical record for Patient #1 on 07/12/21 at 10:00 a.m. with S#1 in a conference room revealed that the medical record for Patient #1 who was discharged from the hospital on [DATE] did not contain a discharge summary.

During a telephone interview with S#9 and in the presence of S#1 on 07/12/14 at 10:16 a.m., S#9 confirmed that the medical record for Patient #1 who was discharged from the hospital on [DATE] did not contain a discharge summary.
