Based on observation, interview and policy review, the hospital failed to ensure all staff don gloves prior to providing care to a patient in contact isolation. This affected one (Patient # 2) of six patients that were in contact isolation. The hospital census at the time of the survey was eight.

Findings include:

Tour of the 14 bed facility was completed on 3/24/14 at 9:10 AM. Outside of room 914 was a sign that read, "Contact Precautions, anyone entering this room MUST wear gloves and gown." A cart next to the door to room 914 contained a supply of gloves and disposable gowns. A staff member (Staff D) was observed at 9:15 AM in the patients room 914 with a gown but no gloves on. Staff D was uncovering the patient's breakfast tray and cutting up the patient's food with ungloved hands. The Infection Preventionist (Staff A) who accompanied the surveyor on tour verified the observation and confirmed the staff member should have gloves on.

The Infection Control policy # IC-201 was reviewed on 3/24/14. The policy gives the rationale for the use of gloves during contact with the infected patient. The policy requires the staff wear gloves to provide a protective barrier and reduce the likelihood that microorganisms present on the hands of personnel will be transmitted to patients.
