MAINTENANCE OF PHYSICAL PLANT

Based on observation, staff interview, and documentation review, the facility failed to maintain a clean, safe environment for the patients of one of one patient unit. This had the potential to affect all patients in the facility. The facility census was 18.
Findings include:

During a tour of the second floor inpatient on 11/04/15 between 10:25 AM and 11:25 AM, three bathrooms of patient rooms 203, 206, 208 were observed and the vents had small grids. Within each grid were visible gray/white dust and fine particles. The patient room 221 was observed on 11/04/15 at 11:03 AM with the Director of Clinical Services. The Director of Clinical Services confirmed the room had not yet been cleaned after the patient had been discharged from the facility on the evening of 11/03/15.

In the patient bathroom of room 203, brown dried soiling was observed on the toilet seat. The Director of Clinical Services confirmed room 203 was a room cleaned and ready for an admission. The Director of Clinical Services indicated the patient in the adjoining room used a bedside commode. The vent at the entrance of room 203 had multiple black/brown spots. In patient bathrooms of rooms 207 and 205, brown debris were observed on the edge of the flooring by the baseboard. A gold/pink colored dried stain was observed in the sink in room 208.

In patient room 213, paint was peeling on the wall behind the sink and on the wall behind the bed. A brown stain was on the bottom sheet of the bed. The Director of Clinical Services confirmed this room had been cleaned and was ready for a patient admission. Room 214 was a patient occupied room, and one of the wall tiles was not attached to the wall on the lower edge and was approximately four inches from the wall. In room 222, the bathroom light was not working and there was peeling paint on the wall behind the bed. In the high observation unit with four beds, the cabinet behind the second bed had multiple areas of chipped paint. There also was a hole at the top of the privacy curtain. The wooden door at the entrance of the high observation room, was chipped with layers of the wood gone from the middle to the bottom of the door. Two ceiling vents between the nurses station and the elevator had multiple dark stains black/brown and ceiling tile close to the vents were discolored.

Staff C was present during the tour and confirmed these findings.

Staff E confirmed on 11/05/15 at 9:20 AM, the air vent covers with grids in the corridor and at the nursing station were partially occluded with white dust and fine air particles. Staff E also confirmed the filters of all air conditioning (a/c) units in the patient rooms were to be changed at least annually. The tracking information was requested for review. At the time of exit on 11/05/15, no documented evidence was provided indicating when any a/c filter was last changed in the patient rooms.
