Based upon observations, facility failed to ensure that one hazardous area was protected in accordance with the 2000 edition of the Life Safety Code, NFPA 101 (National Fire Protection Association). Findings include:
Surveyor observed between 9:30 and 10:00 am on 4/14/10 that the soiled utility room in the nursing unit did not have a self closing device in accordance with NFPA 101, 2000: 19.3.2.1. The door shall be self-closing or automatic -closing.

Based upon observations, the facility failed to ensure that signage was in place in areas where oxygen is stored in accordance with the Life Safety Code, NFPA 101, 2000. Findings include:
Surveyor observed that between 9:30 and 10:00 am that outside oxygen storage area did not have non-smoking or no smoking signs in accordance with NFPA 101, 2000: 19.3.2.4 and NFPA 99: 8.6.4.2.
