Based on observations made during the survey of the facility between the hours of 8:00 am and 1:00 pm, while accompanied by the Risk Mgmt. Dir. and Dir. of Plant Ops it was observed that the facility failed to provide the required Exit Signage at two locations in the KITCHEN:
A. Above the door leading into DINING
B. At intersection of walking paths near COOKING area.

Based on observations made during the survey of the facility between the hours of 8:00 am and 1:00 pm, while accompanied by the Risk Mgmt. Dir. and Dir. of Plant Ops it was observed that the facility failed to provide the following requirements:
1. The air handling units are believed to be greater than 2,000 cfm. If so, then the required minimum smoke detector for the supply air side was missing on all of the air handling units. It was observed that there were smoke detectors on the return air side on all of the air handling units. (Re: NFPA 90A - INSTALLATION OF AIR-CONDITIONING AND VENTILATING SYSTEMS)
2. Log indicating consistent, systematic compliance on: Electrical power and distribution and grounding systems - Effectiveness.
