The facility failed to follow established policies and procedures regarding the transfer/transport of patients for 1 of 10 patient Medical Records (MR's) reviewed (patient P10).

Findings include:

1. Facility procedure for Policy I.C.73, Revised 2/2019, "Transfer and Transport of a Patient," indicates that procedures for transfer require "report to be called to the facility the patient is transferring to." In the case of transport a "Continuation of Care" form would be completed before a patient leaves to be provided to the receiving facility, and upon the patients arrival back at the facility F1.

2. No verbal report or written medical documentation was provided by F1 to F2 on transfer/transport on 2/12/2020. When P10 was returned to F1 from F2 at 1530 hours on 2/13/2020, no documentation of medical treatment status from F2 was found in the MR.

2. On 3/3/2020 at 1600 hours, S3, Chief Executive Officer, and S1, Interim Director of Nursing, confirmed that no documentation was available of verbal report or written supporting medical documentation being provided to the transfer/transport receiving facility F2 on 2/12/2020, and no medical treatment documentation from F2's transfer/transport back to F1 on 2/13/2020 was found in P10's MR.
