Based on document review and staff interview, the hospital failed to provide hospital and unit specific orientation and training to agency employees prior to them working the hospital floor and providing direct patient care. This deficient practice has the potential for inadequate patient care for all who receive care from agency staff.
Findings:
On 03/03/17 a review of policy number HR404.1, titled "General/Department orientation", " ... all new staff members will participate in a formal orientation program to learn the fundamental workings of the facility ..." The policy also documented employees must receive training in the areas of infection control, safety and environment of care before providing patient care.
Documentation review on 03/03/17 included 14 personnel files which included four agency staff members (Staff O, P, T and V). Four of four agency personnel files failed to include hospital orientation, training, or competencies.
An interview with the DON (Director of Nursing) was conducted on 03/03/17 at 11:10 AM, The Director of nursing stated agency staff receive a brief orientation of the medication management system and of the unit's general lay out but no other training was provided and no hospital competencies were required.
