Based on observation of the patient rooms and interview, it was determined the patient care areas had not been maintained and areas of damage in the rooms could cause harm to the patients. This had the potential to affect all patients.
Findings include:
During a tour of the patient rooms conducted 4/26/16 at 11:10 AM with Employee Identifier (EI) # 1, Chief Nursing Officer and EI # 3,Chief Executive Officer the following were observed and confirmed:
Room 107: Two beds that share one bathroom. Room was unoccupied and ready to receive patients. There was a toilet bowl brush in a caddy under the sink in the bathroom. There was no indication if the brush was clean or dirty.
Room 111: Sharps container on the wall just inside the room contained syringes past the Full Line with syringes visible in the open cap.
Room 114: Broken and missing floor tile behind the toilet. The room was unoccupied, clean and ready to receive patients however there was a Bedside Commode in this room with no indication if it was clean or not.
Room 117 and 118 shared bathroom: There was a toilet bowl brush in a caddy under the sink with no indication if it was clean or dirty.
Hall Shower # 1: There was a Mary Jane chair in the shower room with no indication that it was clean. There was no mechanism to indicate the hall shower had been used and needed to be cleaned nor a mechanism to indicate the shower had been cleaned.
Hall Shower # 2: There was a shower chair with no indication if it was clean. There was a partially used bottle of surgical scrub and Aloe Vista Foam wash in the shower. There was no mechanism to indicate the hall shower had been used and needed to be cleaned nor a mechanism to indicate the shower had been cleaned.
Rooms 209 and 210 shared bathroom: Heavy presence of rust on the grate of the heating/cooling unit, cracked tile between the commode and the wall, and grout around the commode was black.
Rooms 211 and 212 shared bathroom: Heavy presence of rust on the grate of the heating/cooling unit, cracked and missing tile between the commode and the wall, and grout around the commode was stained a dark color.
Room 212: Torn wallpaper by the bed approximately 6 inches long and also pulling away from the electrical outlet.
Room 213: Torn wallpaper and a hole in the wall approximately 4 inches in diameter.
Rooms 212 and 213 shared bathroom: Heavy presence of rust on the grate of the heating/cooling unit, the sink had multiple cracks, shower drain had black grout, and black around the base of the wall.
Room 216: Torn wallpaper with multiple holes in the wall behind the bed.
Rooms 215 and 216 shared bathroom: Heavy presence of rust on the grate of the heating/cooling unit.
Rooms 217 and 218 shared bathroom: Presence of rust on the grate of the heating/cooling unit.
Rooms 219 and 220 shared bathroom: Presence of rust on the grate of the heating/cooling unit.
The surveyor observed a sign that read, "Clean toilet, shower chairs, shower stall with Virex TB before and after each use..." The surveyor asked EI # 1 if the sign was meant for the staff or a patient to do and the response was, "the staff but the sign does not say that". The surveyor asked how a patient would know if this was meant for the patient to do and the response was, " the sign does not say".
A tour of room 216 was conducted on 4/28/16 at 8:45 AM with EI # 1. The surveyor observed the holes in the wall behind the bed were patched.
An interview was conducted on 4/28/16 with EI # 4, Plant Operations Director. The surveyor asked when were the patches to the wall in room 216 completed and the response was the evening of 4/26/16. The surveyor requested the initial work order for the holes in the walls in room 216. EI # 4 submitted a work order dated 12/2/15. The surveyor asked why the holes in the wall had not been fixed before 4/26/16 and the response was, "The work order fell through the cracks."
