Based on observation, it was determined that the facility failed to ensure that smoke barriers are constructed to provide at least a one half hour fire resistance rating.

Findings include:

1. On 10/5/14 at 2:00 PM, in the presence of Staff #5, a visual inspection above the ceiling in the corridor outside the Cafeteria provided evidence that the smoke barrier was made of one half inch plywood.

a. One half inch plywood does not have a one half hour fire resistance rating.

Based on observation, it was determined that the facility failed to ensure that doors in smoke barriers self-close.

Findings include:

On 10/4/14 at 2:00 PM, in the presence of Staff #5, a fire rated self-closing door in the Attic was wedged open preventing the door from automatically closing.
