MAINTENANCE OF PHYSICAL PLANT

Based on review of documentation, interview and facility tour, it was determined that the hospital was not maintained in the manner to assure the safety and well being of staff and patients.

Findings were:

"OSHA/Bloodborne Pathogen Regulations Policy #138-030-060" stated in part "The facility provides sufficient housekeeping and maintenance personnel to maintain the interior and exterior of the facility in a safe, clean, orderly, and attractive manner."

Photographs were provided to the surveyor of water leaks from the ceiling and under the walls. Hospital tour on 2/25/25 found water damage on the hallway ceiling and the storage area (not housing patient supplies and not in a patient area) had black mold patches on the baseboards. The Director told the surveyor that leaks happened whenever it rained.

Emails and text messages dated 6/11/24, 7/5/24, 7/8/24, 8/30/24,11/3/24, 11/8/24, 11/11/24, and 11/22/24 were reviewed. These emails and messages included pictures of the water leaks and were sent to the administrative staff of the Exceptional Community Hospital.

The above water damage was confirmed by the Facility's Director on February 25, 2025.
