Based on record review on April 15, 2013, the contractor inspection documents for the fire alarm system stated the system was not in compliance with requirements as follows:
(a) Based on observation on April 15, 2013, a "yellow tag" inspection form, dated January 2012, from a licensed contractor was attached to the fire alarm control panel to document several smoke detectors did not comply with the sensitvity requirments.
(b) Documentation from a licensed contractor fire alarm inspection dated April 5, 2013, states several smoke detectors were not in compliance with sensitivity requirements; several audible and visual alarm signals did not activate as required when tested; the fire sprinkler system water flow device and main control valve supervisory device did not operate properly when tested.

Based on observation on April 15, 2013, the fire sprinkler system did not comply with NFPA Standard 13 at the following areas:
(a) the four operating rooms (OR1, OR2, Cysto OR, Cesearan Delivery OR) in the surgical suite were not protected by the fire sprinkler system.
(b) the "rolling" file cabinets in the medical records suite were too close to the ceiling and obstructed the sprinklers.

Based on observation on April 15, 2013, a "red tag" inspection form from a licensed contractor was attached to the kitchen range hood extinguishing system to document the system was not interconnected with the fire alarm system as required.

Based on record review on April 15, 2013, documnetation for a contractor inspection and maintenance of the emergency generator stated the battery charger was malfunctioning.
