Based on review of records and interview with staff, the facility failed to respond to the grievance/complaint from patient #1 per facility policy and procedure.
Findings:
Policy " EVENT REPORTING : LOR 300 Effective Date: 01/01/2020
It is the policy of the Hospital that any employee, or member of the medical staff (physicians are encouraged to report incidents, but do not fill out incident reports), should report any incidents, accidents or near-misses involving the care of patients and/or any incident related to employees or visitors.
All incidents are to be document and investigated, and the information analyzed, in an effort to resolve an untoward incident or improve processes and/or systems that may be causative factors related to untoward event(s) or incident."
In an interview with staff # 1 December 11, 2024, it was confirmed there was a patient complaint reported via phone and/or text to the facility related to patient care and nonfunctioning elevators that was not documented per policy and procedure.
