Based on observation and interview, it was determined that the facility failed to ensure that the corridors were separated from use areas by walls constructed with at least ½ hour fire resistance rating. In sprinklered buildings, partitions are only required to resist the passage of smoke.
Findings were:
During a tour of the facility by the FSCO and staff member "M", observations revealed the following penetrations:
1. The 6th floor communication room;
2. The 5th floor communication room;
3. The 4th floor janitorial room was missing a ceiling duct vent cover;
4. The old communication room was missing the ceiling tiles; and
5. The 2nd floor janitorial room 370.

Based on observation and interview, it was determined that the facility failed to ensure that exit components (such as stairways) were enclosed with construction having a fire resistance rating of at least one hour, were arranged to provide a continuous path of escape, and provided protection against fire or smoke from other parts of the building.
Findings were:
During a tour of the facility by the FSCO and staff member "M", observations revealed that:
1. Stair West "A" 4th floor had an open conduit penetration; and
2. Stair South "B" 2nd floor had a penetration.
On interview, staff member "M" said that he/she was unaware of the problem.

Based on observation and interview, it was determined that the facility failed to ensure that portable fire extinguishers were provided in all health care occupancies in accordance with code.
Findings were:
During a tour of the facility by the FSCO and staff member "M", observation revealed that the "K" extinguisher type in the kitchen did not have a use placard posted above the extinguisher.

Based on observation and interview, it was determined that the facility failed to ensure that the cooking facilities were protected in accordance with code.
Findings were:
During a tour of the facility by the FSCO and staff member "M", observation revealed that the hoods' pull stations were not labeled as to which station operated which hood system.

Based on observation and interview, it was determined that the facility failed to ensure that the electrical system was maintained per code.
Findings were:
During a tour of the facility by the FSCO and staff member "M", observation revealed that the electrical panels did not have proper labels or covers:
1. Electrical panel 6H2 was not properly labeled;
2. Electrical panel 4H2 was not properly labeled; and
3. Electrical panel 2LK had no cover on the panel.
During a tour of the facility by the FSCO and staff member "M", observation revealed that several pressure type CO2 bottles were not secured by a chain.
