Based on observation, interview and record review, the facility's contracted staff failed to wash hands while holding a contaminated dialysis supply. Staff # A
Findings:
Patient #22
Observation on 07/17/2014 at 01:45 p.m. of Contract Registered Nurse (Staff A) revealed he was observed in the room of Patient #22. The Surveyor asked him to show the dialyzer ' s identity, he grabbed the dialyzer immediately without wearing pair of gloves, and then showed it to the Surveyor. He returned the dialyzer to the Hemodialysis machine ' s clip without washing hands immediately.
The Surveyor immediately notified him that he did not wear gloves, and he missed washing hands right away. He responded " Oh, yes I need to do it."
Review of the facility's Policy and Procedure # H-IC 02-006 titled: Hand Hygiene adopted by the dialysis contracted service directed staff as follows: " Policy hand hygiene will be performed as follows: #D: Before and after patient contact. #F After situations during which microbial contamination of the hands is likely to occur (i.e. contact with potentially contaminated environmental surfaces)".
