Based on visual observation the facility failed to provide 1 of 3 exits that are accessible. This would allow for the exits to not be used in the case of an emergency and potentially affecting 5 of the 5 residents of the facility.

Findings:
During facility tour on 11/18/2010 between the hours of 1:00pm and 4:30pm, the rear exit down from the nurses station did not lead to a public way. Exit stops at a grass area five feet from the exit door.
