Based on observation and interview with staff, the facility failed to maintain Stairways and smokeproof towers used as exits are in accordance with 7.2. 19.2.2.3, 19.2.2.4 Findings,
The door in stair well exit discharge on the ground level on the north end was scotched open.

Based on observation, review of Fire Alarm Report and interview with Safety Officer , the facility failed to provide a fire alarm system required for life safety that is installed, tested, and maintained in accordance with NFPA 70 National Electrical Code and NFPA 72. Findings,

The smoke damper located on the north wall of the entrance of the Hospital on the 4 th floor was not tested during the annual inspection of the Fire Alarm.

Based on observation and interview with staff, the facility failed to provide Electrical wiring and equipment that is in accordance with NFPA 70, National Electrical Code. 9.1.2 Findings,

The room located across from the nurses station (ROOM 475) was occupied with a patient that had 7 pieces of electrical equipment connected. A power strip was draped on the floor with wires creating a trip hazard. The room is not provided with enough electrical receptacles outlets in the wall.

The power strip had not been electrical safety tested and documented in accordance with NFPA 99 1999 edition chapter 7-6.3.1.2.

Because the power strip had not been electrical safety tested the staff failed to remove from service and follow the scheduled maintenance program for electrical equipment 7-6.2.2.2.
