Findings:

a. Based on medical record, policies and procedures review and staff interviews, nursing staff failed to notified physician of change in condition of pressure ulcer on patient #1 and request wound care evaluation from Wound Care Center

b. As per Director of Nursing there is a facility ' s pressure ulcer protocol when a change in condition is noted a request for wound care evaluation is done. Staff fail to notified physician of change of condition on pressure ulcer, no wound care evaluation was order. Patient was discharge with out wound care evaluation. Facility's staff failure to follow standard protocol for pressure ulcers put patient at risk.
