Based on findings from document review and interview, 1 of 2 medical records (MRs) reviewed lacked documented evidence a patient activities assessment was performed and 2 of 2 MRs reviewed each lacked documentation of an activities plan of care, as required by both hospital policy and this regulation.

Findings include:

--Per review of the hospital policy entitled "Activities Program," last revised 3/08, it states "The Activities Director will complete a patient assessment and give input into the interdisciplinary care plan in respect to diversional and social activity interests."

--Per review of Patient A's MR, he/she was a swing bed patient from 11/23/09 - 12/8/09. There was no evidence that an activities assessment or activities plan of care was completed for Patient A during his/her swing bed hospitalization.

--Per review of Patient B's MR, he/she was admitted to the swing bed program on 2/10/10. As of the date of this survey, 2/18/10, there was no documentation in the MR regarding an activities plan of care.

--Per interview of the Activities Director on 2/18/10 at 3:15 p.m., and the Swing Bed Nurse Manager on 2/18/10 at 4:30 p.m., the above findings were acknowledged.
