1. Based on an interview with the facility's administrator and review of facility policy- the facility failed to comply with established process for prompt resolution of patient grievances as per facility policy and as required by regulation.

The findings included:

a. Based on a interview with facility administrator on 27 November, 2012 in the administration conference room, he stated that a letter was mailed to complainant on 01 June, 2012 which was over 20 days after complaintant submitted greivance, he stated according to policy the facility should have responded within 14 days but failed to do so.
b. Reviewed facility policy titled Resolution of Patient Complaints/Grievances on 28 Novemember, 2012 in the facility conference room and found that facility should have provided complainant with information on who to contact but provided wrong information, provided information to contact the Idaho Department of Health and Welfare as per policy. The facility also did not abide by the timeline of notifying the complainant within 3 working days of the status of grievance or provide any other information on own required timeline.
