Based on review of facility documents and staff interviews (EMP) it was determined that Brookville Hospital failed to follow acceptable standards for temperature control in the Surgical Suite for 39 out of 69 days.

Findings:

Review of "Brookville Hospital Policy: Surgical Services: Number: SUR-018, ... Revised: 4/09 ... Title; Preventive Maintenance. Purpose: To provide safety and comfort within the Surgical Suite for both patients and staff. Policy: Environmental parameters are established for Operating Room suites and rooms are maintained to assure that the Operating Room parameters are met. Variances of the ranges, action and resolutions are documented and reported. Procedure: 1. The humidity and temperature is recorded daily during normal weekday hours of operation. When the parameters are exceeded, the Maintenance Department and the Surgical Service Director is notified. a. Recommended range for temperature in the Operating Room is 60 - 73 degrees. ... F. ... 4. If any problems arise, Maintenance is notified immediately. ... ."

1. A review of the "Temperature Log" for Operating Rooms 1, 2 and 3, revealed that for the month of March 2012, the documented temperatures were outside of the acceptable range for 39 of 69 days.

2. During an interview with EMP1 on April 10, 2012, at approximately 2:30 PM, "some of the physicians like the temperature to be lower. We will put the temperature range (68 to 73 degrees F.) on the "Temperature Logs".
