Based on observation and interview the facility failed to provide care in a safe setting. This deficiency is evidenced by toilets with ligature risk points at the hinged toilet seat and at the base of the tank behind the toilet bowl in 20 of 20 (a-f) toilets located on the unit for patient use.
Findings:

Tour of the unit on 05/23/2022 between 2:45 p.m. and 3:55 p.m. revealed the restrooms in a, b, d, f, h, i, q and r had hinged operable toilet seats and space below the tank and behind the bowl which could be used as ligature points. S1ADm and S2DON verified the findings during the tour.

During the tour at 3:35 p.m. S2DON verified that all the toilets on the unit were the same and all were ligature risks.

Based on observation and interview the facility failed to ensure the premises were maintained at an acceptable level and quality. This deficiency is evidenced by failure to maintain 2 of 2 (q and r) shower rooms at an acceptable level of cleanliness.
Findings:

Tour of the facility on 05/23/2022 a between 2:45 p.m. and 3:55 p.m. revealed shower room q and shower room r each consisted of 3 showers and a toilet area. The showers in both shower rooms were noted to have rust colored crusty material along the exterior edges of the showers and pink/ orange staining of the grout in various areas in all the showers.

In interview on 05/23/2022 at 2:50 p.m. and 3:40 p.m. S1Adm and S2DON verified the showers were not properly maintained.
