DISCHARGE PLANNING - EARLY IDENTIFICATION

Based on record review and interview, the hospital failed to identify at an early stage of hospitalization those patients who are likely to suffer adverse health consequences upon discharge if there is not adequate discharge planning, in that, 10 of 10 patients (Patient #1,#2, #3, #4, #5, #6, #7, #8, #9 and #10) did not have an assessment of their discharge planning at the early stage of hospitalization.

Findings:

Review of medical records on 3/3/2016 in the Board Room of the hospital with Personnel #1 failed to show a nursing assessment of discharge plans at the patient's admission to the hospital for 10 of 10 patients (Patient #1, #2, #3, #4, #5, #6, #7, #8. #9, and #10.)

During an interview with Personnel #1 on 3/3/2016 in the Board Room of the hospital, Personnel #1 was asked if there was a nursing assessment that documented the patient's plan for discharge. She answered, "No."

DISCHARGE PLANNING EVALUATION

Based on record review and interview, the hospital failed to provide discharge planning evaluation for patients identified for post hospital services and to other patients upon the patient's request, the request of a person acting on the patient's behalf, or the request of the physician, in that 10 of 10 patients (Patient #1, #2, #3, #4, #5, #6, #7, #8, #9 and #10) did not have discharge planning evaluations for post hospital services.

Findings:

Record review on 3/3/2016 in the Board Room of the hospital with Personnel #1 failed to document any discharge planning evaluations for post hospital services for 10 of 10 patients (Patient #1, #2, #3, #4, #5, #6, #7, #8, #9, #10).

During an interview with Personnel #1 on 3/3/16 in the Board Room of the hospital, Personnel #1 asked if she could show any discharge planning for 10 of 10 patients (Patient #1, #2, #3, #4, #5, #6, #7, #8, #9, and #10). She answered, "No."
