Facility failed to maintain HVAC systems in accordance with the 2000 edition of the Life Safety Code, NFPA 101. Findings included the following:

Surveyor observed between 10:30 and 11:30 am, on 12/13/11 that HVAC systems were not functioning in the Lobby and Emergency department. Further it was noted that no HVAC was supplied to the patient wing corridor. This was confirmed by the facility manager.

Surveyor between 10:30 and 11:30 am on 12/13/11 that fire extinguisher did not have a placard in accordance with NFPA 10: 4.3.2.2. The placard shall read "fire protection system shall be activated prior to using fire extinguisher.

Facility failed to maintain electrical receptacle testing in accordance with the 1999 edition of the Standard for Health Care Facilities, NFPA 99. Findings included the following:

Surveyor observed on 12/13/11, between 1:00 and 1:30 pm, that no records of receptacle testing were available. Receptacles shall be tested in Patient Care areas as follows:

Phyisical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shal be not less than 4 oz. (test)

Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.
