Facility failed to maintain smoke barrier rated wall integrity at two corridor crossings in accordance with the 2000 edition of the Life Safety Code, NFPA (National Fire Protection Association) 101. Findings included the following:

Surveyor observed between 1:00 and 2:00 pm on 4/22/14 that at 2 places in the corridor between facility and clinic and in the corridor leading to imaging that open penetrations were not adequately sealed above the ceiling.

Facility failed to maintain hazardous area in accordance with the Life Safety Code NFPA 101. Findings included the following:

Surveyor observed between 1:00 and 2:00pm on 4/22/14 that room in inactive nursing corridor leading to dietary was used for storage was in excess of 50 sqs ft and did not have self-closing device on door to corridor.

Facility failed to maintain kitchen hood in accordance with NFPA 96. Findings included the following:

Surveyor observed between 1:00 and 2:00 pm on 4/22/14 that the inside of the ktichen hood was greasy to the touch.
