Based on observations during the survey walk of the facility on the afternoon of 11/18/2015, while being accompanied by the Chief Executive Officer and Director of Risk Management, the facility failed to provide the direction of the egress path. No exit sign existed in the egress corridor at the control doors near assessment room.

Based on observations during the survey walk of the facility on the afternoon of 11/18/2015, while being accompanied by the Chief Executive Officer and Director of Risk Management, the facility failed to provide correct labeling of fire alarm panel.
Fire Alarm Control Panel (FACP) did not appear to be correctly cross labeled with the electrical panel that provides the power for the FACP. The FACP panel shall have a permanent label on the face of the FACP panel that indicated the electrical panel and circuit number that supplied power to the FACP.
NFPA 72; 2002: 4.4.1.4.2.3 ..... The location of the circuit disconnecting means shall be permanently identified at the fire alarm control unit.
Additionally, the circuit breaker that provides power to the Fire Alarm system shall be marked with a red color within the panel to meet the requirements of NFPA 72(2002)1-5.2.5.2
